Source of Funds Forms 

Acorn Life must comply with the requirements of the Criminal Justice (Money Laundering and Terrorist Financing) Act 2010, as amended, to obtain certain information relating to the Source of Funds for payments made to Acorn Life.

The below Source of Funds forms are required for use for certain payments and depending on the customer type.

Individual Customers

Fund Switch and Redirection Form

If Policy Owner(s) wish to proceed without financial advice and effect fund switch or redirection changes, a Fund Switch Form and Redirection Form must be completed by each Policy Owner.

It is recommended Policy Owner(s) obtain appropriate financial advice prior to making fund switch or redirection changes to a policy. If Policy Owner(s) require financial advice they should contact their Financial Advisor or contact Acorn Life Customer Care at info@acornlife.ie  or 1800 446 446 to be referred to a Financial Advisor.

For encashment requests on policies, please use the Policy Encashment Options Form. Completing this form is the most effective way for us to process your request. If you are a resident living outside of the Republic of Ireland, you must provide a Declaration of Residence form.
